To get started with a Home
and Garden Party Candle Fundraiser for your group, please
click on the link below to open the agreement form. Once
opened, you can print the form, fill it out and fax it back to
me at
1-801-454-5510. You may also
return the agreement form by postal mail to the address listed
on the form. You will receive a phone call within 48 hours to
verify the information on the form and answer any final
questions you may have. If you have any problems opening and
printing the agreement form, please
email me or call me today
at 1-860-482-3976. I'd be happy to fax an Agreement Form
to you.
Please note that by submitting an
Agreement Form you are agreeing to the terms outline and you
anticipate submitting an order with 20 items or more.
Should your fundraiser be cancelled you'll agree to pay 20
cents per brochure plus reimburse all postal charges
incurred to send you the brochures.
Once your Agreement Form has been
submitted your brochures will be sent by USPS priority mail.
